General Information - Name/Address tab
Enter the participant's name and address on this window.
How to Access the General Information – Name/Address Tab
On the Setup menu, point to Participants and then click General Information. The Participants General Info Details window displays.
How to Add a Participant
1. Click the Add icon to display a new row for data entry.
2. Complete the following required fields on the Name/Address tab:
· Participant ID
· Last Name
· First Name
· SSN
3. Complete any of the optional fields.
4. Click the Dates tab.
5. Complete the following required field on the Dates tab:
· Birth date for the participant
6. Click the Password tab.
7. Complete the following required fields on the Password tab:
· Web Login ID
· Web Password
8. Complete any other optional information for the participant.
9. When you are done, click the Save icon.

Participant ID |
Enter a Plan participant's identification number. This will be the primary system identifier for a participant. It will be used for all system searches and reports as well as VRU and Participant Site access. Note: For security purposes, you may decide to use something other than a Social Security number as a Participant ID. |
